First Appointment

If you are scheduled for your first WIC appointment, please bring:

    • Proof of Address

    • Proof of Income

    • Proof of Identification

    • If you are pregnant, please provide proof of pregnancy

Examples of acceptable forms of proof are listed below.

Please note: You will receive a text message from us asking you to upload your proofs before your appointment. If you cannot upload the documents via the link sent to you, please bring the documentation with you to the appointment. 
 
You will also need to bring with you any children under the age of 5 that you plan to enroll in the WIC program. If you are unable to bring your child to the WIC office, please call 1-888-999-6897.

If possible and applicable to you, please also bring your child’s immunization record and blood test results (Hgb/Hct) from your doctor or clinic, along with any documents that you weren’t able to upload prior to the appointment.

Proof of Address

Acceptable examples of proof of address include:

  • Any mail or postcard

  • Bill

  • Income tax return

  • CA State Driver’s License or ID card, or pay stub.

Proof of Income

Acceptable examples of proof of income include:  

  • Income Tax Return

  • Pay Stub for Wages or Salaries

  • Unemployment Insurance

  • Many times, evidence of enrollment in other programs (example, Medi-Cal card or a letter from CalFresh or TANF) can qualify you for WIC.

Proof of Identity for all eligible family members

Acceptable examples of proof of identity include:

  • Medi-Cal Card

  • Birth Certificate

  • Immunization Record

  • Driver’s License

  • Photo ID

  • Medical Referral Form

  • Social Security Card

  • Aid Verification Letter/Notice of Action.

Proof of Pregnancy

Acceptable examples of proof of pregnancy include:

  • Ultrasound with name and date

  • Medical Records stating pregnancy

  • Written pregnancy verification from health care provider

  • Medical referral form

  • Prenatal blood work request form with name and date

  • Prenatal appointment notice with name

  • A letter demonstrating presumptive eligibility for prenatal Medi-Cal

  • Prescription for prenatal vitamins
