How to Apply for WIC
Find out if your family might qualify for the WIC program, how to enroll and what to expect at your first appointment.
A. To qualify for WIC a person must be in one of the following categories:
*A pregnant woman
*A woman who was pregnant within the past 6 months
*A woman who is breastfeeding a baby under 1 year of age
*A baby or child from birth to his or her fifth birthday**
**Babies and children cared for by a single father, grandparent, foster parent, step-parent or guardian are also eligible for the WIC Program
B. To qualify for WIC a household must also meet Income Guidelines. A person may also qualify if they receive Medi-Cal, CalWorks (TANF) or CalFresh (SNAP) benefits.

If you qualify for the WIC program, the WIC representative will schedule an enrollment appointment for you at one of our 11 locations.
Attend your enrollment appointment with the paperwork needed to verify your eligibility.
We will verify WIC eligibility
*proof of address
*proof of income
*proof of identification
*If you are pregnant, please bring proof of pregnancy.
Examples of acceptable forms of proof are listed in the table below. Please note: You can bring or show these forms of proof in an electronic format, for example, a photo on a cell phone or an email attachment.
Please also bring:
*any children under the age of 5 that you plan to enroll in the WIC program. If you are unable to bring your child to the WIC office, please call 1-888-999-6897.
If possible and applicable to you, please also bring:
*your child’s immunization record
*blood test results (Hgb/Hct) from your doctor or clinic.
What To Bring
|Proof of Address||Acceptable examples of proof of address include: any mail or postcard, bill, income tax return, CA state driver’s license or ID card, or pay stub.|
|Proof of Income||Acceptable examples of proof of income include: income tax return, pay stub for wages or salaries, or unemployment insurance. Many times, evidence of enrollment in other programs (example, Medi-Cal card or a letter from CalFresh or TANF) can qualify you for WIC.|
|Proof of Identity for all eligible family members||Acceptable examples of proof of identity include: Medi-Cal card, birth certificate, immunization record, driver’s license, photo ID, medical referral form, social security card, or aid verification letter/notice of action.|
|Proof of Pregnancy||Acceptable examples of proof of pregnancy include: Ultrasound with name and date, medical records stating pregnancy, written pregnancy verification from health care provider, medical referral form, prenatal blood work request form with name and date, prenatal appointment notice with name, a letter demonstrating presumptive eligibility for prenatal Medi-Cal, prescription for prenatal vitamins|

You will participate in a nutrition assessment including measurement of Height and Weight

To prepare, if you received Nutrition Questionnaires in the mail, please complete them at home and bring them with you.
