Here are the things that you will need to bring to your first appointment along with examples of acceptable forms of proof:

Proof of Identity for all eligible family members Acceptable examples of proof of identity include: Medi-Cal Card, Birth Certificate, Immunization Record, Driver’s License, Photo ID, Medical Referral Form, Social Security Card, or Aid Verification Letter/Notice of Action.
Proof of Address Acceptable examples of proof of address include: any mail or postcard, bill, income tax return, CA state driver’s license or ID card, or pay stub.
Proof of Income Acceptable examples of proof of income include:  Income Tax Return, Pay Stub for Wages or Salaries, or Unemployment Insurance. Please call 1-888-999-6897 to see if your enrollment in other programs (such as Medi-Cal, CalFresh, or TANF) can qualify you for WIC.
Eligible Children Bring the children under age 5  that you plan to enroll in the WIC program. If you are unable to bring your child to the WIC office, please call 1-888-999-6897.

  • An Individualized Nutrition Assessment for each qualifying member of your family
