What to Bring
Here are the things that you will need to bring to your first appointment along with examples of acceptable forms of proof:
What To Bring
|Proof of Identity for all eligible family members||Acceptable examples of proof of identity include: Medi-Cal Card, Birth Certificate, Immunization Record, Driver’s License, Photo ID, Medical Referral Form, Social Security Card, or Aid Verification Letter/Notice of Action.|
|Proof of Address||Acceptable examples of proof of address include: any mail or postcard, bill, income tax return, CA state driver’s license or ID card, or pay stub.|
|Proof of Income||Acceptable examples of proof of income include: Income Tax Return, Pay Stub for Wages or Salaries, or Unemployment Insurance. Please call 1-888-999-6897 to see if your enrollment in other programs (such as Medi-Cal, CalFresh, or TANF) can qualify you for WIC.|
|Eligible Children||Bring the children under age 5 that you plan to enroll in the WIC program. If you are unable to bring your child to the WIC office, please call 1-888-999-6897.|

- An Individualized Nutrition Assessment for each qualifying member of your family
